LURUS CREDIT CARD AUTHORIZATION FOR AUTOMATIC BILLING
If you elected  to pay your account by credit card or debit card, before your account can be activated you  must agree to the following: terms and conditions of  the automatic billing agreement for credit card payment.
It applies to all accounts  under your account name, once initiated, until canceled in writing with Lurus. Lurus will bill  your credit card or debit card, entered below, based on the subscription term selected on your online application for Internet access. Your credit or debit card will be charged from the date your account becomes active until Lurus discontinues service to your account or until you  request, by e-mail, in writing via US Postal mail or by fax, that automatic billing be discontinued.

1. Lack of use of your account during the billing period does not release you  from your payment obligation.

2.This agreement does not relieve you of your obligation to  pay for Lurus Internet services if any charge submitted by Lurus is dishonored, charged-back  or otherwise refused for any reason. Any such event will be treated as a returned check and  will be assessed a processing charge of $25.00 for each occurrence. You agree to pay all  reasonable collection charges and costs, including attorney's fees and expenses of collection if payment is dishonored.

3. Lurus reserves the right to require you to re-certify automatic billing to us on our forms at intervals determined by the organization(s) that the firm uses for  merchant processing.

4. If automatic billing is canceled for any reason, you must complete and  sign another Authorization for Automatic Billing form to Lurus to reactivate your automatic  billing feature. Updates will not be accepted over the telephone under any circumstance

5. Lurus Internet bills on a monthly basis. Lurus does not prorate it's rates or services except, at its discretion for the rate drop if activation occurs after the 23rd of any month. Therefore,  cancellation requests must be received by Lurus before 5 p.m. on the last day of the month to avoid charges for the next billing cycle NO EXCEPTIONS! Cancellations MUST be made in  writing by US Postal mail, by e-mail, or fax.

6. Completion of a separate Authorization for  Automatic Billing form is required for each account that is to be billed by credit card or debit  card. No changes may be made to this agreement. You agree that Lurus may accept any  altered agreement as if the changes had not been made.
